Serving NYC, Westchester and the tri-county area since 1972, Sprain Brook Manor has grown into a magnificent 121-room facility. Since its founding, its mission has been to provide clients with the highest level of physical and occupational therapy, skilled nursing care and rehabilitative services.

Akiva Fried, LNHA, LMSW
Akiva Fried, LNHA, LMSW

Administrator

As both a licensed health care administrator and social worker with a specialization in gerontology, Akiva has dedicated his entire career to improving quality of life and well-being for the geriatric population. Akiva brings many years of leadership experience to Sprain Brook Manor Rehab, with passion and empathy serving as his guiding principles every step of the way. In April 2015, Akiva joined the Sprain Brook Manor “dream team” (as he calls it) with a primary focus on ensuring residents are provided with the highest quality level of individualized resident-centered care.

Amy Mendizabal

Director of Nursing

Amy has been with Sprain Brook Manor for the past eight years. Amy graduated “Cum Laude“ with a B.SN. from the University of Aquinas College of Nursing in the Philippines (now University of St. Thomas). Soon after, she immigrated to the United States to pursue her dream of becoming a nurse. She has had a long and distinguished career in healthcare, and her diverse experience makes her a valuable member of our team. Amy started as a Clinical Instructor and then went on to serve as a Charge Nurse, an RN Supervisor, an In-service Coordinator and as Rehab Nursing Director before being named Director of Nursing.

Client Services Specialist

A lifelong resident of Yonkers, Joni started at Sprain Brook in 2010 and quickly moved up through the ranks to Client Services. Her official title may be Client Services Specialist, but she also helps plan many of our events and serves as the purchasing director for all non-clinical supplies for both Sprain Brook and our sister facility, Adira at Riverside.

Juan Carlos “JC” Coats

Director of Activities

JC received his B.S. from Hofstra University with a degree in Business Management and a minor in Marketing. While working a side job with his dad as a porter in a nursing home, he captured the attention of the activities director. She saw a spark in JC, took him under her wing and showed him the ropes. From the moment he got involved with the senior residents and learned about all aspects of the activities department, JC was hooked and has never looked back. He will be furthering his education this fall at Lehman College, where he will pursue a Masters in Therapeutic Recreation.

Almethia Middleton
Almethia Middleton

Director of Social Work

Almethia Middleton is a graduate of Yeshiva University and The College of New Rochelle. She began her social work career advocating for children within the foster care system. In 2013 she transitioned into senior care. Her background as a CNA gives her a unique perspective to working with the senior population. In addition to being passionate about her work, Almethia is devoted to her loving family.
