Sprain Brook Manor was formed 38 years ago to meet the challenges facing the senior long term care industry. Located in the lovely Scarsdale, NY, operates a 121 state-of-the-art skilled nursing and rehabilitation center. Sprain Brook Manor is not a typical nursing home; we are continually upgraded with major renovations to create more luxurious senior living accommodations and to provide superior residential care. Cardiac Rehab

Sprain Brook Manor takes cardiac rehab to the next level by providing short term, comprehensive care to facilitate a resident's quick return home. Our therapy team treats residents recovering from cardiac conditions, such as heart attack, heart surgery or stable angina, or other cardiac conditions, with a focus on cardiac surgery recovery.

The program is specifically designed for residents who are unable to return home immediately after cardiac surgery and who need medical supervision and intensive rehab to ease their transition back into the community. Physician involvement from area hospital physicians specializing in cardiopulmonary disease is the foundation of this program.

Respite Care

When your loved one requires senior care, you want to help in every way possible. Unfortunately, there are times when personal limitations or unforeseen circumstances prevent you from providing the appropriate care at home. Sprain Brook Manor can help.

Respite care is for families who prefer to care for a loved one in a home setting but want or need a break. Especially helpful when your loved one is very ill or unstable, this care option gives you, the caregiver, time to recover from an often overwhelming, stressful experience. Respite care can be as short as a few days or as long as several weeks; it is completely your choice. Our admissions team works with the caregivers, the resident, and the resident's doctors to assess care needs.

Caring for the caregiver is one of the most important aspects of sustaining an extended home care situation. As a caregiver, you may be unwittingly ignoring signs of exhaustion, illness, or depression in an effort to care for your loved one. Stress and sleep deprivation contribute to caregiver burnout, causing persistent symptoms of depression, anxiety, irritability and anger. Sprain Brook Manor's staff offers comfort and support to the friends and family of each resident to help alleviate this stress. We understand the impact that the warmth of a caregiver can have and offer personalized, compassionate senior long term care, post acute care, and rehab.

Short Term Rehab

Sprain Brook Manor's short term rehab services are offered seven days per week. Our programs help residents fully recover from surgery, serious injury, or illness and return home as soon as possible with the skills to perform daily living tasks. Sprain Brook's physical therapy team develops personalized care plans for each resident based on adaptive equipment, pre-discharge home evaluations, and resident and family instruction.

Distinguished in senior rehab care, Sprain Brook's Medical Department includes primary care physicians, specialists, skilled nurses, and a complete roster of consultants.

Stroke Rehabilitation

Sprain Brook Manor's skilled nursing and post acute care units offer excellent care for individuals recovering from stroke-related disability. All services work on a continuum of restoring the stroke survivor's function, providing compensation strategies and adaptation of the environment. Stroke recovery services include computer-assisted cognitive and vocational training, community-based training, and home evaluation.

Our Occupational Therapy team helps stroke survivors restore their daily activities, such as dressing, bathing, cooking, and managing money. Our Speech Therapy team helps restore their critical ability of communication, assisting with expressing themselves and understanding others. These therapists also treat swallowing and eating abilities, as well as impairments in cognition such as judgment, reasoning and problem solving. Our Physical Therapy team helps stroke survivors restore mobility from their wheelchairs or walkers to independent ambulation.

PAYMENT OPTIONS

Medicare & Medicaid

Medicare is a federal health insurance program for people 65 and over and people with certain disabilities under age 65. To be eligible to receive Medicare coverage, residents must require skilled nursing care or rehabilitative care on a daily basis. They must have spent three consecutive nights in a hospital and be admitted to a skilled nursing home within 30 days of discharge from the hospital. Also, a physician must certify that the services required from the skilled nursing home are needed for the same illness for which the resident was hospitalized.

Medicare Part A covers care provided by a skilled nursing home to help residents recover from an acute illness or injury. Medicare provides complete coverage for the first 20 days of skilled care and a portion of the cost for days 21 to 100. During this time frame, residents are required to pay a daily coinsurance rate.

Skilled nursing home services covered by Medicare include:

  • Meals
  • A semi-private room
  • Regular nursing services
  • Rehabilitation therapy services
  • Drugs furnished by the facility
  • Medical supplies

Medicaid is a health insurance program in which the federal and state governments share the cost of payments to nursing facilities on behalf of eligible residents. Medicaid is for low-income people 65 and over, people who are disabled, people who are blind, and people who need nursing home care. Some people qualify for both Medicaid and Medicare; and Medicaid is sometimes used to help pay for Medicare premiums. People who qualify for both programs are called 'dual eligible'.

To be eligible to receive Medicaid coverage, residents must meet a state-determined poverty level and certain health criteria. Because Medicaid is based on financial need, extensive information on income and assets is required for qualification.

For qualifying individuals, Medicaid pays for nursing facility services in certified skilled nursing homes. Skilled nursing home services covered by Medicaid include:

  • Meals
  • A semi-private room
  • Regular nursing services
  • Rehabilitation therapy services
  • Drugs furnished by the facility
  • Medical supplies

Additional insurance is available to supplement Medicare or Medicaid coverage, to pay for additional services. Residents with such insurance coverage should bring a copy of their policy to their local Sprain Brook Admissions Staff so that they can determine how to receive the most benefit.

Residents can apply for Medicare at their local Social Security office. Residents can apply for Medicaid at their local Department of Health office or at their state's Medicaid agency.

Private Pay / Insurance

Private Pay is the primary funding source for most long term care products and services. This refers to costs that are not covered by Medicare, Medicaid or private insurance and are therefore borne by the resident or his/her family. Individuals without long term care insurance and who do not qualify for Medicare or Medicaid must pay for these services themselves.

Sprain Brook Manor is also a qualified provider of nursing facility care and services under many third party private insurance companies. If a resident is entitled to benefits under insurance offered by a non-Medicare/non-Medicaid insurance program, and if we participate as a provider under their program, Wingate will accept payment from the program. The resident will, however, be responsible for paying all co-payments, co-insurance, deductibles and charges for items and services that the program does not cover.
